La thrombopénie induite par l’héparine est un état clinico-biologique complexe lié à une héparinothérapie, véritable standard anticoagulant et responsable, paradoxalement, d’un état pro-thrombotique majeur. Elle représente la plus grave des complications iatrogènes de l’héparinothérapie qu’il convient de savoir diagnostiquer précocement et de traiter efficacement. Sa prise en charge constitue une véritable urgence thérapeutique nécessitant le relai par des agents antithrombotiques non hépariniques tels que le danaparoïde sodique et les inhibiteurs directs de la thrombine visant à inhiber l’hypercoagulabilité induite par la génération explosive de thrombine. Le diagnostic biologique peut être parfois difficile malgré les progrès accomplis dans la compréhension de son mécanisme et le développement des tests immunologiques et fonctionnels ayant des performances diagnostiques et de nombreux avantages. Un des tests fonctionnels les plus performants est le test de libération de la sérotonine radiomarquée (SRA) qui est historiquement considéré comme le test de référence.
