L’Uganda dipende fortemente dai pagamenti diretti (OOP) per finanziare la propria assistenza sanitaria, ma i dati dimostrano che oltre il 10% della spesa lorda delle famiglie incide negativamente sul loro benessere. Maggiore interesse è stato rivolto alla determinazione dell’entità, della distribuzione e dei fattori determinanti della spesa sanitaria catastrofica e alle modalità per farvi fronte. Lo studio mirava a stabilire l’effetto che i pagamenti diretti hanno sul benessere delle famiglie. Utilizzando i dati dell’indagine nazionale sulle famiglie dell’Uganda del 2005/06, lo studio ha rilevato che il 28,1% delle famiglie in Uganda sostiene spese sanitarie catastrofiche, distribuite tra tutti i gruppi socioeconomici, ma in misura maggiore tra le famiglie rurali. Tale spesa riduce il benessere delle famiglie o aumenta la povertà del 5,8%. Sebbene la spesa alimentare sia protetta, l’istruzione e l’alloggio ne risentono. Lo studio chiede al governo di proteggere le famiglie dalle spese sanitarie catastrofiche attraverso assicurazioni e sistemi prepagati e di aumentare il reddito delle persone per migliorare la loro capacità di pagare le cure. La metodologia di calcolo della povertà dovrebbe essere adeguata per tenere conto delle spese sanitarie ed evitare una sottovalutazione. I risultati saranno utilizzati dai responsabili delle politiche di finanziamento della sanità, dagli economisti sanitari e dai pianificatori.
