La citometria a flusso è una tecnica sensibile ampiamente utilizzata in diversi campi, tra cui l’onco-ematologia, l’immuno-ematologia e l’emostasi. Lo studio della funzione piastrinica consente di individuare le malattie ereditarie legate a disturbi delle glicoproteine, come la trombastenia di Glanzmann e la sindrome di Bernard Soulier. L’analisi si basa sul passaggio delle piastrine davanti a uno o più laser dopo essere state marcate con vari fluorocromi. Le piastrine subiscono cambiamenti morfologici durante l’attivazione, nonché cambiamenti nell’espressione di alcune glicoproteine, sia presenti sulla loro superficie allo stato nativo sia contenute nei loro granuli. Questa tecnica può essere utilizzata anche per monitorare i pazienti sottoposti a terapia antiaggregante e per individuare malattie immunologiche come la trombocitopenia indotta da eparina e la porpora trombocitopenica immunologica. La citometria a flusso è la tecnica d’elezione per lo studio delle alterazioni piastriniche, in quanto offre un’eccellente sensibilità e riproducibilità, ma richiede tecniche standardizzate e un operatore performante.
