I funghi predominanti isolati dalla bocca umana appartengono al genere Candida e, sebbene esistano più di 150 specie di Candida, circa solo 10 di queste colonizzano la cavità orale. Candida albicans (C. albicans) è la specie più diffusa (70-75% degli isolati), seguita da C. glabrata e C. tropicalis (7% degli isolati). Una serie di fattori, tra cui l’ospite, sistemici e iatrogeni, è stata associata a un aumento dell’incidenza di infezioni opportunistiche che coinvolgono la Candida, considerata quindi una specie patogena. La transizione di questo innocuo commensale in 'parassita' che causa malattie può essere associata agli attributi di virulenza del microrganismo e a una serie di fattori dell’ospite. I fattori che contribuiscono alla patogenicità includono le molecole di superficie che mediano l’adesione e l’invasione nelle cellule dell’ospite, la secrezione di idrolasi, la transizione lievito-alfa, il contact sensing e il tigmotropismo, la formazione di biofilm, il cambiamento fenotipico e una serie di attributi di fitness. In questa sede vengono illustrate le varie modalità con cui questo normale commensale diventa patogeno, le possibili interazioni con le cellule epiteliali dell’ospite e il ruolo nella trasformazione maligna della mucosa orale.
