Os fungos predominantes isolados da boca humana pertencem ao género Candida, e embora existam mais de 150 espécies de Candida, aproximadamente apenas 10 destas colonizam a cavidade oral. A Candida albicans (C. albicans) é a espécie mais prevalente reconhecida (70 a 75% dos isolados), seguida da C. glabrata e da C. tropicalis (7% dos isolados). Uma série de factores, incluindo o hospedeiro, sistémicos e iatrogénicos, tem sido associada a um aumento da incidência de infecções oportunistas envolvendo Candida e, por conseguinte, consideradas como espécies patogénicas. A transição deste comensal inócuo para um 'parasita' causador de doenças pode estar associada aos atributos de virulência do microrganismo e a uma série de factores do hospedeiro. Os factores que contribuem para a patogenicidade incluem as moléculas de superfície que medeiam a adesão e a invasão das células hospedeiras, a secreção de hidrolases, a transição de levedura para alfa, a deteção de contacto e o tigmotropismo, a formação de biofilmes, a mudança fenotípica e uma série de atributos de aptidão. Aqui apresentamos uma panorâmica das várias modalidades de como este comensal normal se torna patogénico, as suas possíveis interações com as células epiteliais do hospedeiro e o seu papel na transformação maligna da mucosa oral.
