The educational spiral in medical training aims to foster the acquisition of professional competence. Although theoretical knowledge and technical skills are essential for practice, clinical competence also encompasses many other attributes, such as interpersonal skills, the ability to gather relevant data during the history-taking and physical examination, problem-solving skills, and clinical reasoning. However, while active learning methods have been introduced, current assessment of learners does not take into account clinical reasoning in contexts of uncertainty. The script concordance test is a reliable, objective, valid, and convenient tool for assessing clinical reasoning in contexts of uncertainty. It does not replace other assessment instruments but should be used in conjunction with them.
