La spondylarthrite ankylosante (SA) est une maladie auto-immune rhumatismale chronique touchant les articulations de la colonne vertébrale et les tissus mous adjacents. Cette pathologie survient chez le sujet jeune et prédomine chez les hommes. Des facteurs génétiques et environnementaux sont suspectés dans la genèse de la maladie, notamment la vitamine D, son récepteur (VDR) et l’HLAB27.L’évolution clinique et le pronostic dépendent de l’âge d’apparition dont la survenue tardive se caractérise par une élévation marquée des paramètres biologiques et une atteinte grave, comparativement au début précoce.Les différences de manifestations cliniques selon le sexe pourraient être attribuées à des combinaisons variables de certains haplotypes génétiques. De plus, les patients présentent fréquemment des manifestations extra-articulaires (EAM), qui constituent des éléments importants pour un diagnostic approprié. Parmi celles-ci, l’uvéite, les maladies inflammatoires chroniques de l’intestin (MICI), où elle est présente dans environ 2 à 9 % des cas, ainsi que le psoriasis.
