Este livro destaca o impacto do real em um hospital. Fundamenta-se na Psicanálise, nas abordagens teóricas de Sigmund Freud e de Jacques Lacan, trazendo também contribuições de outros autores. Aponta, inicialmente, os três registros indicados por Lacan – o simbólico, o imaginário e o real – dando ênfase ao real, que se manifesta, frequentemente, no contexto hospitalar. O real é definido como o que escapa ao simbólico, o que não tem sentido e pode chegar a ser traumático. O conceito de trauma é abordado enquanto um excesso de energia que invade o aparelho psíquico, sem que o sujeito consiga se preparar para poder dar conta dele. Como possíveis proteções contra o real são estudadas a angústia, as fantasias e o humor. Constata-se que as presenças constantes da angústia e da morte no hospital testemunham que o real está ali, insistindo em aparecer. A angústia enquanto afeto que surge quando o sujeito se confronta com o real, e a morte como uma das figuras do real, aquela que aponta para o impossível. Diante de um perigo, por exemplo, o da iminência da morte, muitos pacientes se angustiam. Eles estão, constantemente, frente a acontecimentos da ordem do horror, do insuportável. Também será discutido o discurso médico, predominante no hospital, e o discurso do analista, enfocando as diferenças no modo como eles lidam com a presença do real no hospital. São apresentados diversos fragmentos de casos clínicos, a fim de articular a minha prática com a teoria de Freud e de Lacan. 10
