Os metabolitos endógenos do ácido araquidónico com propriedades semelhantes aos compostos da Cannabis sativa Linnaeus, os chamados endocanabinóides, têm efeitos em vários tipos de cancro. O principal ingrediente ativo da cannabis, o Δ9-tetrahidrocanabinol (Δ9-THC), produz os seus efeitos através da ativação dos recetores CB1 e CB2. Os recetores CB1 são expressos em níveis elevados no sistema nervoso central (SNC), enquanto os recetores CB2 estão concentrados predominantemente, embora não exclusivamente, nas células do sistema imunitário. Embora os endocanabinóides e os canabinóides sintéticos possam ter efeitos pró-proliferativos, foram descritos efeitos predominantemente inibidores no crescimento tumoral, angiogénese, migração e metástase. Excecionalmente, estes efeitos podem ser seletivos para as células cancerígenas, enquanto as células e tecidos normais são poupados. Essa aparente seletividade das células tumorais torna o sistema endocanabinóide um alvo potencial atraente para a terapia do cancro. Portanto, são necessárias mais pesquisas básicas sobre as propriedades anticancerígenas dos canabinóides, bem como ensaios clínicos da eficácia terapêutica dos canabinóides no cancro da mama, da próstata e dos ossos.
