I metaboliti endogeni dell’acido arachidonico con proprietà simili ai composti della Cannabis sativa Linnaeus, i cosiddetti endocannabinoidi, hanno effetti su vari tipi di cancro. Il principale principio attivo della cannabis, il Δ9-tetraidrocannabinolo (Δ9-THC), produce i suoi effetti attraverso l’attivazione dei recettori CB1 e CB2. I recettori CB1 sono espressi ad alti livelli nel sistema nervoso centrale (SNC), mentre i recettori CB2 sono concentrati prevalentemente, anche se non esclusivamente, nelle cellule del sistema immunitario. Sebbene gli endocannabinoidi e i cannabinoidi sintetici possano avere effetti proliferativi, sono stati descritti prevalentemente effetti inibitori sulla crescita tumorale, l’angiogenesi, la migrazione e le metastasi. Eccezionalmente, questi effetti possono essere selettivi per le cellule tumorali, mentre le cellule e i tessuti normali vengono risparmiati. Tale apparente selettività delle cellule tumorali rende il sistema endocannabinoide un potenziale bersaglio interessante per la terapia del cancro. Sono quindi necessarie ulteriori ricerche di base sulle proprietà antitumorali dei cannabinoidi, nonché studi clinici sull’efficacia terapeutica dei cannabinoidi nel cancro al seno, alla prostata e alle ossa.
