Focault argumenta que a imagem, ou construção teórica, da natureza da 'loucura' - com os seus componentes físicos, mentais e sociais - precede a etiologia, o diagnóstico e o tratamento da loucura. Esta construção persiste mesmo quando é profundamente imperfeita e sem provas científicas.
