Bei 30 - 50 % der Patienten in Klinik und Praxis liegen behandlungs­ bedUrftige Erkrankungen oder Gesundheitsstorungen vor, die im engeren oder weiteren Sinn als emahrungsabhangig gelten mUssen. Diatetische Therapie, frtiher orientiert an Kriterien wie Schonung oder 'Schutz' ('Leberschutzkost'), basiert in zunehmendem MaBe auf gesicherten wissenschaftlichen Erkenntnissen und ist imstande, Stoff­ wechselstorungen mittels Kontrolle der Nahrungszufuhr zu kompen­ sieren und ihre pathophysiologischen Folgen zu verhindem oder wenigstens zu mildem. Beispiele sirid die verminderte Zufuhr von Nah­ rungsbestandteilen, deren Stoffwechsel gestort ist (gewissen Amino­ sauren bei Phenylketonurie, Nahrungsfett bei Typ I-Hyperlipoprotein­ amie), oder der Eingriff in den EiweiBhaushalt bei der hepatischen Enzephalopathie nach portocavalem Shunt bzw. der Azotaemie bei Niereninsuffizienz. Mit der Zunahme der Haufigkeit emahrungsabhangiger Gesundheits­ storungen hat sich deren Spektrum verschoben. Emahrungsabhangige Mangelkrankheiten (z.B. Skorbut) haben ab-, Folgen der Dber- oder Fehlemahrung wie Fettstoffwechselstorungen, Diabetes, Hypertonie und Gicht erheblich zugenommen. Dem Zuwachs neuer Erkenntnisse Uber Indikationen und Wirkungs­ weise bestimmter Emahrungsformen stehen allerdings erhebliche Schwierigkeiten bei der Durchfuhrung der als richtig erkannten MaB­ nahmen in der Praxis entgegen, die nicht bagatellisiert werden dUrfen.
