Ils se développent dans des zones essentielles à la respiration, à la parole, à la déglutition et à la perception sensorielle, et leurs effets se font sentir dès les premiers instants. Mais derrière chaque tumeur visible se cache un univers invisible d’événements moléculaires - perturbations génétiques, altérations épigénétiques, voies de signalisation aberrantes et changements microenvironnementaux - qui convergent pour transformer des cellules normales en cellules malignes. Ce livre commence dans cet espace invisible, là où l’oncogenèse prend racine. Au cours des dernières décennies, notre compréhension de la formation et de l’évolution des cancers de la tête et du cou a été radicalement modifiée par les progrès de la science. Historiquement considérés principalement en termes d’anatomie et d’histopathologie, ces cancers apparaissent aujourd’hui comme des maladies dont les voies de causalité moléculaires sont bien définies et détectables. L’identification du papillomavirus humain à haut risque comme moteur d’un sous-type unique de cancers oropharyngés, l’élucidation des paysages mutationnels à l’aide du séquençage à haut débit et la caractérisation des interactions tumeur-immunité ont contribué à réécrire l’histoire des oncogènes.
