Os bacilos Gram-negativos não fermentativos são uma ameaça crescente para a saúde pública. Este estudo, realizado num hospital universitário no centro da Tunísia, forneceu um perfil epidemiológico e bacteriológico das estirpes isoladas entre 2019 e 2022. As principais espécies foram Pseudomonas aeruginosa (51,8%), Acinetobacter baumannii e Stenotrophomonas maltophilia, representando em conjunto 93,5% dos isolados. A resistência intrínseca da Pseudomonas aeruginosa e a sua capacidade de adquirir novos mecanismos de resistência ilustram a necessidade urgente de estratégias terapêuticas inovadoras. A evolução das estirpes deAcinetobacter baumannii e Stenotrophomonas maltophilia durante a pandemia de COVID-19 revela uma tendência preocupante. A utilização maciça de antibioticoterapia empírica está a favorecer o aparecimento de estirpes altamente resistentes, o que faz temer impasses terapêuticos. Por último, a ocorrência inesperada de infecções por Comamonas, nomeadamente de peritonite, sublinha a necessidade de uma vigilância acrescida face às espécies emergentes e ao seu impacto clínico nos doentes vulneráveis.
