L’evoluzione del virus dell’epatite C (HCV) è un processo altamente dinamico. Esistono poche informazioni sull’epidemiologia molecolare degli isolati di HCV in Marocco, un’area nota per una prevalenza intermedia dell’infezione da HCV. Lo scopo di questo studio era determinare la distribuzione dei sottotipi dei ceppi di HCV in pazienti con infezione cronica da HCV in Marocco e stimare la prevalenza delle sostituzioni aminoacidiche nella regione core dell’HCV in pazienti marocchini non trattati in precedenza. In questo studio abbiamo incluso 185 pazienti positivi all’anti-HCV. L’identificazione del sottotipo di HCV è stata effettuata rispettivamente mediante sequenziamento delle regioni 5’UTR, core e NS5B. La storia demografica dell’HCV è stata dedotta utilizzando un’analisi bayesiana con catena di Markov Monte Carlo. L’analisi filogenetica basata sulla regione NS5B ha rivelato che il sottotipo 1b (75,2%) era il più comune, seguito dal sottotipo 2i (19,1%). Il sottotipo 1b dell’HCV aveva una prevalenza ancora più elevata nel carcinoma epatocellulare. Utilizzando un approccio bayesiano, la data media di comparsa dell’antenato comune più recente è stata stimata nel 1910 per l’HCV-1b e nel 1854 per l’HCV-2i. Sulla base della regione core, sono state rilevate mutazioni a R70Q o L91M in più di un quarto dei pazienti infetti da HCV-1b.
