L’RNA messaggero è diventato il fulcro della ricerca in medicina molecolare, in quanto è una molecola non tossica che consente l’espressione transitoria di caproteine in praticamente tutti i tipi di cellule, comprese le cellule dendritiche. I principi alla base dei vaccini a base di acidi nucleici sono radicati nel dogma centrale di Watson e Crick: il DNA viene trascritto in mRNA, che a sua volta viene tradotto in proteine. L’mRNA viene prodotto sinteticamente e formulato in vaccini. Nella sintesi dei vaccini a mRNA, i trascritti di mRNA vengono purificati mediante cromatografia liquida ad alte prestazioni (HPLC) per rimuovere i contaminanti e i reagenti. Gli mRNA vengono tradotti negli antigeni corrispondenti dopo essere stati inoculati nelle cellule dell’ospite, imitando l’immunità umorale e cellulare simile a un’infezione virale. Con l’aiuto del vaccino a mRNA è possibile aumentare la reattività dell’antigene. I vaccini a mRNA ottimizzati devono soddisfare il requisito della piena attivazione dell’immunità innata per avviare l’immunità adattativa. Il primo candidato vaccino lanciato in sperimentazione clinica è un vaccino a base di mRNA somministrato tramite nanoparticelle lipidiche. Questo vantaggio determina la rapida risposta immunitaria dopo la somministrazione di mRNA. Il vaccino a mRNA è ampiamente utilizzato per le malattie infettive.
