O glioblastoma e as metástases cerebrais são os tumores mais comuns do sistema nervoso central em adultos. É importante fazer a distinção entre estas duas entidades no pré-operatório, uma vez que requerem abordagens terapêuticas e acompanhamento diferentes. A diferenciação entre glioblastoma e metástases cerebrais é um desafio radiológico, particularmente em doentes sem origem neoplásica conhecida e com uma lesão cerebral solitária. O objetivo deste estudo é ilustrar o valor da RM multimodal na diferenciação entre glioblastoma e metástases cerebrais.Para as sequências morfológicas, os critérios mais distintivos foram: localização, número, infiltração de NGCs e CC, hipersinal Flair cortical peri-tumoral sem realce e o rácio entre o hipersinal Flair peri-lesional e o tamanho do realce tumoral. Para a sequência de perfusão, a medição do rCBV intra e peri-tumoral no GBM e MC revelou uma diferença estatisticamente significativa entre os dois tipos de tumor.
