Le gel flottant in situ est l’un des systèmes d’administration de médicaments gastro-rétenteurs qui représente une révolution dans les formes posologiques à libération contrôlée par voie orale par rapport aux liquides oraux conventionnels. Ils prolongent le temps de séjour du médicament qui a une fenêtre d’absorption étroite dans les sites d’absorption comme l’estomac ou le tractus gastro-intestinal supérieur (GI), car ils ont une densité apparente inférieure à celle des fluides gastriques. Ils restent donc en suspension dans l’estomac sans affecter la vitesse de vidange gastrique jusqu’à ce que tout le médicament soit libéré en continu à un rythme lent. Le furosémide est un diurétique de l’anse à plafond élevé largement utilisé chez les patients souffrant d’insuffisance cardiaque congestive (ICC) pour éliminer l’excès d’eau corporelle, réduire la pression artérielle et mobiliser les œdèmes. Cette étude a porté sur la formulation d’une solution orale de furosémide qui se gélifie au contact direct du liquide gastrique et qui flotte en utilisant soit des polymères primaires comme l’alginate de sodium et la gomme gellane, soit en combinaison avec des polymères secondaires comme le carraghénane iota et l’HPMC. Différentes évaluations ont été effectuées sur les 35 formules de gel in situ pour mesurer la force du gel, le temps de gélification, l’indice de gonflement, l’uniformité du contenu, le temps de latence du flottement, la durée du flottement, le pH et la densité.
