Les anticorps anti-centromères (ACA) peuvent être détectés par immunofluorescence indirecte (IFI) ou par immunodot (ID). Notre objectif était d’évaluer la concordance IFI/ID et la relevance clinique des ACA.Les patients présentant une positivité des ACA par IFI sur cellules Hep-2 et/ou par ID ont été inclus. Les ACA étaient détectés dans 63 cas (2,1 % des tests IFI+ID). Les ACA étaient positifs par IFI et ID dans 18 cas. Pour les discordances, il s’agissait de 2 cas ACA-IFI+/ACA-ID- et 43 cas ACA-IFI-/ACA-ID+. Pour ces 43 cas, l’aspect de la fluorescence nucléaire (IFI) était moucheté (34), moucheté nucléolaire (6) ou homogène (3); les ACA (ID) étaient faiblement positifs (26), positifs (7) ou fortement positifs (10).Le diagnostic de connectivité (sclérodermie systémique, lupus érythémateux systémique, polyarthrite rhumatoïde) a été retenu chez 3 patients parmi 18 ayant des renseignements cliniques. L’IFI sur cellules Hep-2 a certaines limites (opérateur-dépendante ; un aspect de fluorescence peut être masqué par un autre). L’ID est utile pour confirmer la spécificité antigénique, dans la limite des antigènes utilisés. L’interprétation de ces tests repose sur le contexte clinique.
