Introdução: Na Costa do Marfim, foi adoptada uma estratégia de isenção de taxas de utilização dos serviços de saúde materna nos estabelecimentos públicos, a fim de eliminar o obstáculo financeiro ao acesso aos cuidados. O objetivo do nosso estudo foi avaliar a prestação de cuidados de saúde materna gratuitos do ponto de vista das famílias. Métodos: De maio a julho de 2022, realizámos um estudo transversal descritivo de mães selecionadas por amostragem sistemática que tinham um bebé com menos de um ano de idade.Resultados: Um total de 486 mães com uma idade média de 29,63±6,59 anos. Cerca de 3 em cada 10 mulheres não frequentavam a escola e 42% tinham uma atividade geradora de rendimentos. Mais de 90% dos pagamentos diretos destinavam-se a cesarianas (50/50), ecografia (377/384), medicamentos (360/384) e laboratório (344/384). O item mais caro foi a hospitalização, com uma média de 149395,60 ± 32497,35 FCFA, e a cesariana com 140600 ± 59776,99 FCFA.Conclusão: A isenção de taxas não é efectiva nos serviços de saúde materna. A indisponibilidade de certos serviços constitui um obstáculo à aplicação desta disposição.
