Neste livro foi explicado claramente o papel das células imunitárias na rejeição de aloenxertos. Tanto os mecanismos de destruição tecidular específicos do antigénio como os não específicos caracterizam as respostas de rejeição aos aloenxertos de pele, embora seja a ação específica do antigénio das células Tc que determina principalmente a sobrevivência ou a rejeição da pele enxertada. As respostas de rejeição de aloenxertos de pele são iniciadas in vivo pela ativação de células TH reactivas a antigénios por células de Langerhans do dador residentes no enxerto. Parece ser possível promover a aceitação do aloenxerto através da inativação selectiva de células TH reactivas ao antigénio ou de células Tc através de vários protocolos experimentais diferentes. As células TH podem ser inactivadas por exposição ao aloantigénio na ausência de células TH ou dos seus produtos. A auto-tolerância pode ser induzida através da utilização de muitos dos anticorpos monoclonais, células dendríticas e moléculas HLA-G. Até mesmo o quimerismo misto e a tecnologia anti-sentido estão a ser amplamente utilizados para o desenvolvimento da tolerância e são normalmente utilizados vários fármacos para suprimir o sistema imunitário, que podem ser específicos ou não específicos.
