O hipotiroidismo é um dos distúrbios da tiroide mais frequentes em todo o mundo. O músculo esquelético é um dos principais órgãos-alvo da hormona tiroideia e são necessários níveis normais de hormona tiroideia para manter a função normal e a integridade celular dos músculos esqueléticos. O envolvimento muscular no hipotiroidismo é comum, com 30-80% dos doentes com hipotiroidismo a apresentarem sintomas musculares que variam de mialgia a miopatia verdadeira. Apesar da intensa investigação, a miopatia no hipotiroidismo permaneceu enigmática até há pouco tempo, mas é uma causa importante de morbilidade na maioria dos doentes com hipotiroidismo. Reconhece-se um padrão de elevação das enzimas musculares séricas no hipotiroidismo, com enzimas como a CK, a LDH e as aminotransferases frequentemente elevadas devido à miopatia associada. O nosso estudo concluiu que a elevação significativa das actividades séricas de CK, LDH e SGOT indica um envolvimento muscular no hipotiroidismo e que estes ensaios enzimáticos podem ser utilizados para o rastreio e o diagnóstico precoce de miopatia subjacente em doentes com hipotiroidismo. Além disso, estas enzimas (CK, CKMB, LDH e SGOT/AST), juntamente com T3, T4 e TSH, podem ser utilizadas para detetar a gravidade do hipotiroidismo.
