La polyarthrite rhumatoïde (PR) est une maladie auto-immune complexe dont la pathogenèse est fortement liée à la génétique. Les facteurs génétiques sont responsables de la majorité de la susceptibilité de la population à cette maladie. L’antigène leucocytaire humain (HLA) est un groupe de gènes situé sur le bras court du chromosome 6 entre les bandes 6p21.31 et 6p32. Il s’étend sur un segment d’ADN d’environ 4 000 kbp et représente environ 1 % du génome humain. Le complexe HLA contient plus de 200 gènes, dont plus de 40 codent pour des antigènes leucocytaires. Il est polygénique. Il existe plusieurs gènes pour chaque classe de molécules et il existe un grand nombre d’allèles polymorphes dans la population pour chacun des gènes. Les molécules HLA sont soupçonnées de jouer un rôle dans la pathogenèse de maladies auto-immunes telles que la PR, le diabète insulinodépendant et la sclérose en plaques.
