De acordo com esta abordagem, o hegemon é visto como benigno na medida em que proporciona estabilidade e segurança para que outros beneficiem e prosperem sob a sua liderança, ao mesmo tempo que sacrifica os seus interesses de curto prazo em prol da estabilidade do sistema. Neste livro, o autor contesta esta teoria utilizando a teoria neo-gramsciana que foi introduzida nos anos 70 por Robert Cox com base nos escritos de Antonio Gramsci sobre o conceito de hegemonia e ideologia. O livro centra-se na política externa dos EUA no Médio Oriente devido à importância desta região para os interesses materiais dos EUA e da classe hegemónica dominante. Assim, o caso do Médio Oriente pode ser considerado ideal para compreender como a ideologia hegemónica das elites foi aplicada, e continua a ser, para salvaguardar esses interesses.
