Jeśli chodzi o powikłania, osteoporoza powoduje co roku ponad 8,9 miliona złamań na całym świecie, z czego ponad 4,5 miliona ma miejsce w Stanach Zjednoczonych i w Europie. W ostatnich latach coraz większą uwagę poświęca się różnicom między płciami w zakresie osteoporozy i związanych z nią złamań.
