Eine erfolgreiche Nierentransplantation bietet eine bessere Lebensqualität und -dauer und ist für Patienten mit chronischer oder terminaler Nierenerkrankung medizinisch und wirtschaftlich wirksamer als eine Langzeitdialysetherapie. MMF wird als Antiproliferationsmittel der ersten Wahl für die anfängliche Erhaltungstherapie bei Nierentransplantatempfängern empfohlen. Mycophenolatmofetil (MMF) und magensaftresistentes Mycophenolat-Natrium (EC-MPS) sind beide für den Einsatz in fester Dosierung bei Nierentransplantationen zugelassen. In den letzten Jahren wurde über mehrere große klinische Studien und zahlreiche pharmakokinetische Untersuchungen berichtet, die neue Erkenntnisse in die Debatte über den klinischen Einsatz der therapeutischen Arzneimittelüberwachung (TDM) von MPA bei Nierentransplantatempfängern eingebracht haben. Der Einsatz von TDM für MPA ist jedoch nach wie vor umstritten; einige Transplantationsteams haben bereits routinemäßig Überwachungsprogramme eingeführt, während andere die MPA-Exposition nie messen.Die Pharmakokinetik von MPA ist durch eine hohe Variabilität zwischen den Patienten und eine zeitabhängige Veränderung gekennzeichnet.
