Les tumeurs de la glande pituitaire et de la région sellaire représentent 10 à 15 % de l’ensemble des processus expansifs intracrâniens. Elles regroupent une grande variété de lésions néoplasiques, inflammatoires, vasculaires et dysembryologiques, dont la majorité sont des adénomes hypophysaires (90% des lésions sellaires). Avant la chirurgie, d’autres diagnostics doivent être exclus car la stratégie thérapeutique optimale varie significativement d’une lésion à une autre. L’objectif de ce travail était de décrire les pathologies non adénomateuses et de dresser leur tableau clinique et radiologique classique afin de proposer la stratégie thérapeutique optimale. Les lésions sellaires non adénomateuses sont des pathologies rares et leur étude minutieuse nécessite un perfectionnement des techniques exploratrices radiologiques et biologiques et une sous-spécialisation dans la chirurgie sellaire, dans le but d’affiner le diagnostic préopératoire et d’établir le meilleur plan thérapeutique.
