Le diagnostic des lésions buccales repose sur une anamnèse et un examen clinique précis ainsi que sur des examens complémentaires. Le test au bleu de toluidine, l’examen cytologique et la biopsie sont les plus habituels. Le test au bleu de toluidine, tombé en désuétude, consiste à rechercher la présence de cellules atypiques, riches en acide nucléique, et qui captent fortement le colorant. L’examen cytologique ou cytologie exfoliative, est réalisé sur un prélèvement obtenu par grattage de cellules épithéliales étalées sur une lame. Cette technique, facile à réaliser, est indiquée dans des lésions telles que les candidoses ou les lésions bulleuses mais pas dans les lésions précancéreuses en raison de nombreux faux négatifs. Seule la biopsie qui permet d’obtenir une étude histo-pathologique du tissu prélevé à des fins diagnostiques reste l’examen de référence.
