La salud pública se ocupa de la salud colectiva. Trabaja con la incidencia de las enfermedades, con la prevención de estas y con la promoción de la salud. Por ello, no suele atender a pacientes concretos en una consulta, ya que de eso se ocupa la medicina clínica. Y, por esa razón también, los problemas y los conflictos éticos de la salud pública no coinciden con los de la ética clínica. Las políticas de la salud pública, por ejemplo, tienen un objetivo beneficente y no maleficente diferente al de la medicina clínica porque no buscan el bien de un individuo concreto, sino el de la comunidad, lo que supone conflictos potenciales con las libertades individuales que van más allá de la autonomía del paciente y un riesgo potencialmente mayor de daño o maleficencia. También tienen una relación especial con la igualdad porque la salud pública debe garantizar la equidad en la salud, lo que la enfrenta con los conflictos éticos y políticos propios de la justicia social. Por todo ello, la salud pública requiere de una integración ad hoc de la ética en todas sus políticas.A partir de la experiencia y del intercambio de conocimiento práctico, este documento aborda la cuestión y plantea algunas propuestas sobre la gobernanza e institucionalización de la ética en un contexto de nuevos desafíos en la salud pública.
