The Brockville Risk Checklist 4 - Scoring Manual is a scoring guideline document designed to inform clinicians and researchers on how to properly score and use the tool. The BRC4 is a short, dynamic risk assessment instrument that serves a risk management purpose in a forensic mental health facility. It was designed to be used as an ongoing risk monitoring tool to be scored by an interdisciplinary treatment team on a regular basis. The BRC4 measures dynamic risk under three facets - Harm to Others, Harm to Self and Exploitation by Others, while also examining Protective and Neglect factors and tallies a range of objective incidents which occurred over the desired reporting period.This scoring manual includes detailed instructions on how to properly score each item and gives examples for the range of possible selections. It also taps into common misunderstandings and questions that may arise during use, as the authors combined have encountered countless assessment situations with this version of the tool where feedback from clinical staff and author observations were consistently noted to inform the content of this manual. The manual is designed to allow a clinician to properly and effectively use this tool without any outside training necessary.
