When the Ayushman Bharat Digital Mission was concpetualised in 2021, the ambition was audacious: a federated, privacy-first digital health infrastructure for 1.4 billion people. Years later, the architecture exists. The registries are live. The APIs are open. By almost every technical measure, India has built something the world has not seen at this scale. But here is the uncomfortable truth that no dashboard will tell you: infrastructure is not impact. Patients in rural primary health centres still navigate paper. Providers comply because they must, not because it helps. The communities who need digital health most are the least likely to use it. And the metrics that matter, care quality, continuity, financial protection, are nowhere on the scoreboard. From Infrastructure to Impact tears open the gap between what was built and what is actually working. Drawing on policy benchmarking against WHO and OECD frameworks, rigorous health-economic modelling, and twenty unfiltered stakeholder interviews across government, hospitals, frontline providers, and civil society, this assessment asks the question India’s digital health establishment must now answer: what does it take to turn the world’s most ambitious health data system into something every citizen actually feels? The findings are frank. But the opportunity is clear. When inclusion is funded as an investment rather than treated as a subsidy, social returns double. When outcomes replace inputs on national dashboards, accountability follows. When interoperability is mandated at the point of care rather than encouraged at the policy level, the system finally delivers what it promised. ABDM has moved India from policy ambition to architectural reality. This book makes the case for what comes next: a system that works quietly, seamlessly, and for everyone.
