A periodontite é uma doença inflamatória crónica caracterizada pela destruição dos tecidos de suporte dos dentes, principalmente devido a uma resposta imunitária-inflamatória exagerada do hospedeiro ao biofilme microbiano. A terapia periodontal convencional centra-se principalmente no controlo mecânico da placa bacteriana; no entanto, não aborda diretamente a degradação dos tecidos mediada pelo hospedeiro. Esta limitação levou ao aparecimento da Terapia de Modulação do Hospedeiro (TMH) como uma abordagem adjuvante no tratamento periodontal. A terapia de modulação do hospedeiro tem como objetivo alterar ou reduzir a regulação das vias inflamatórias destrutivas, reforçando simultaneamente os mecanismos de proteção e regeneração, restabelecendo assim o equilíbrio entre a defesa do hospedeiro e a destruição dos tecidos. Os recentes avanços neste campo expandiram-se para além da utilização de doxiciclina em doses subantimicrobianas, passando a incluir anti-inflamatórios não esteróides, bifosfonatos, estatinas, ácidos gordos ómega 3 com mediadores especializados pró-resolução, probióticos, antioxidantes e agentes biológicos que visam citocinas e enzimas específicas envolvidas na inflamação periodontal e na reabsorção óssea.
