Foi elaborado pelo Dr. Rishi Raj Singh, Professor Sénior do Instituto Lenora de Ciências Dentárias, em Rajahmundri, AP, Índia. Foram envidados esforços para apresentar uma perspetiva mais clínica, que é frequentemente difícil de compreender e concretizar durante o tratamento de um doente.
