Aggressive periodontitis is a rapidly progressive periodontal disease characterized by rapid attachment loss and alveolar bone destruction, usually affecting young individuals with an otherwise healthy medical history. Its key features include familial aggregation, severe periodontal destruction disproportionate to the amount of plaque, and involvement of pathogens such as Aggregatibacter actinomycetemcomitans and Porphyromonas gingivalis. It is classified into localized and generalized forms: localized aggressive periodontitis mainly affects first molars and incisors, while generalized aggressive periodontitis affects multiple teeth and shows episodic, extensive destruction. Its pathogenesis involves bacterial virulence factors along with abnormalities in host immune responses, particularly neutrophil and phagocyte function. Early diagnosis and treatment are essential to control infection, arrest disease progression, and preserve periodontal support.
